When abdominal fat becomes a risk factor – Modern approaches in orthopaedics for the treatment of obesity
Obesity has long been more than just an aesthetic challenge – it is a medical risk factor with far-reaching consequences for the entire musculoskeletal system. In our orthopaedic practice in Frankfurt am Main, we see patients every day whose excess weight not only restricts their well-being, but also leads to serious illnesses such as slipped discs, osteoarthritis or chronic joint problems. But there is good news: Modern diagnostics and innovative, conservative therapies enable new ways out of the cycle of weight, pain and restricted movement.
Belly fat – the underestimated hormone organ
Many people don’t know that visceral abdominal fat in particular is not just passive fatty tissue – it is highly hormonally active. It produces pro-inflammatory messenger substances (e.g. interleukins, TNF-alpha), which promote systemic inflammatory processes and play a central role in the development of osteoarthritis and back pain.
Excess abdominal fat also exerts mechanical pressure on the spine. The intervertebral discs, which act as a buffer between the vertebrae, are permanently overloaded. The result: premature wear and tear, pain, herniated discs – often at a comparatively young age.
The InBody measurement: precise body analysis instead of BMI
Looking at the scales or BMI is no longer enough to provide targeted and individualized treatment. In our practice, we therefore rely on bioimpedance analysis. This modern technology enables precise measurement of body composition: muscle mass, visceral fat, water balance – all of this can be displayed in detail and documented over time.
This is particularly important for people with obesity: it allows us to identify where exactly fat deposits are located, whether sufficient muscle is present and how the body actually changes during therapy – regardless of the mere kilos on the scales.
Medically assisted fat reduction – when diets are not enough
Not everyone with obesity benefits from traditional methods such as dietary changes or exercise. In certain patient groups – such as those with pronounced metabolic syndrome, insulin resistance or long-term obesity – conventional measures reach their limits.
For these cases, we offer medically supervised therapy with fat removal injections. Unlike cosmetic treatments, these injections are aimed at supporting the metabolism and the targeted mobilization of fat reserves – always under medical supervision and in strict compliance with health criteria.
Lipedema injections contain active pharmaceutical ingredients that influence fat metabolism or regulate the feeling of hunger – they are usually administered subcutaneously. Frequently used drugs include GLP-1 receptor agonists (e.g. tirzepatide – Mounjaro®). Studies have shown that modern preparations are very well tolerated and – if medically indicated – are a promising addition to existing therapies.
An important part of this therapy is the monitoring of specific blood values. The levels of adiponectin and leptin are particularly relevant here:
- Adiponectin: A hormone that has an anti-inflammatory effect and improves insulin sensitivity. An increase indicates a positive metabolic response.
- Leptin: Is produced by fatty tissue and actually signals to the body that sufficient energy is available. Leptin is often elevated in obesity – which paradoxically can be associated with “leptin resistance”. A decrease in leptin levels can indicate an improved hormone balance and a reduction in visceral fat.
